Intensive Case Management Nurse

Position: Intensive Case Management Nurse Role
Join the Island Health team as a Community Outreach Intensive Case Management Nurse. Work within a dedicated mental health and addictions unit, delivering essential care in both community and facility settings.

You will provide critical assessment, treatment, and support to clients with mental disorders as part of a multidisciplinary team.

Your role involves developing tailored client rehabilitation plans while ensuring compliance with relevant health legislation. This position primarily focuses on nurturing client relationships and enhancing their recovery experiences.

Key Responsibilities:

• Conduct assessments and provide mental health treatments

• Design and maintain personalized rehabilitation initiatives

• Monitor medication side effects and efficacy

• Engage with families and community health resources

• Implement crisis intervention techniques when necessary

Requirements:

• BC licensure as an RN or RPN

• At least one year of experience in psychiatric nursing

• Familiarity with Psychosocial Rehabilitation model and interventions

• Valid CPR certification

• Effective communication and teamwork abilities
