The Patient Navigator operates under the guidance of the Manager, specializing in patient assessment and recruitment while actively improving healthcare delivery. This role involves direct clinical care and educational outreach, ensuring that stakeholders are well-informed and supported throughout the patient journey.
Key Responsibilities:
• Facilitate patient recruitment and selection processes
• Implement improvements for service quality and efficiency
• Deliver direct nursing care and comprehensive education
• Serve as a resource for healthcare professionals
• Recommend enhancements to program structures and tools
Requirements:
• Graduate from an accredited nursing program
• Minimum 3 years of clinical experience in relevant fields
• Active RN registration with the BC College of Nurses
• A valid BC Driver’s License is essential
• Preferred certification in program area
