As an RN dedicated to primary care, your responsibility will include delivering comprehensive services to clients through interdisciplinary collaboration. You'll employ a cultural humility approach while advocating for client needs and managing transitions across different health services effectively. Your commitment to best practices will ensure that you contribute positively to health promotion and patient outcomes.
Key Responsibilities:
• Conduct thorough client assessments and health interventions
• Educate families about health management and support
• Collaborate on care plans with multidisciplinary teams
• Ensure culturally safe and trauma-informed practices
• Assist in preventing unnecessary hospital admissions through timely care
Requirements:
• Registered Nurse with BC College of Nurses and Midwives
• At least two years of relevant clinical nursing experience
• Possess a valid BC driver's license for travel
• Certification in Basic Life Support Level C
• Strong ability to navigate healthcare disciplines and policies
