Lead home support services with Island Health as a Licensed Practical Nurse (LPN) supervisor. Oversee Community Health Workers while ensuring quality care and service delivery.
As a Home Support CHW Supervisor at Island Health, you will coordinate and supervise the delivery of home support services. This position requires a collaborative approach with the interdisciplinary team to assess needs and provide direct nursing care. Your expertise in the field will enhance client service and support CHWs through training and evaluation.
Key Responsibilities:
• Supervise Community Health Workers in the field
• Implement and oversee service plans for clients
• Conduct assessments and provide personal nursing care
• Evaluate CHWs' skills and competency in care
• Collaborate with interdisciplinary teams for optimal service
Requirements:
• Valid LPN registration with BC College of Nurses and Midwives
• Three years of related community care experience
• One year of supervisory experience preferred
• Valid BC Driver's License and reliable vehicle
• Strong communication and organizational skills
