As a Home Support CHW Supervisor, you'll play a key role in coordinating care services at Island Health. Your responsibility is to train and evaluate Community Health Workers, ensuring high-quality patient care in collaboration with an interdisciplinary team. Key Responsibilities:
• Oversee CHWs in delivering home support services

Requirements:
• Current LPN registration with BC College of Nurses
• Minimum three years of related care experience
• One year of supervisory experience required
• BC Driver's License and reliable transport
• Effective communication and leadership abilities
