As a Community Liaison Nurse, you will work closely with an inter-professional health care team, focusing on complex discharge planning and coordination for clients in need of community services. Utilizing strong communication and assessment skills, you'll connect with families, primary care providers, and health service organizations. The role ensures continuity of care and supports clients’ specific health needs.
Key Responsibilities:
• Coordinate complex discharge planning for clients and caregivers
• Liaise with healthcare providers and community services
• Evaluate and document client care needs effectively
• Promote continuity of care in all interactions
• Participate in ongoing professional development
Requirements:
• Graduation from a recognized nursing program
• Registration with BCCNM as a practicing RN
• At least three years in gerontology and acute/community health
• Valid Class 5 BC Driver's License
• Strong communication and problem-solving abilities
