In this role, you will work collaboratively with healthcare professionals to plan and assess the care needs of clients transitioning to community-based services. You’ll use your nursing expertise to promote healthy outcomes while fostering relationships with clients, their families, and community partners. Your communication skills will play a crucial role in ensuring effective service delivery.
Key Responsibilities:
• Engage in complex discharge planning for community clients
• Facilitate communication among healthcare teams
• Assess and document client needs accurately
• Foster partnerships with primary care and community services
• Stay informed on community health resources
Requirements:
• Recognized nursing program graduation
• Active BCCNM registration as RN
• Minimum three years in gerontology care
• Valid Class 5 BC Driver’s License
• Proficient in time management and decision-making
Support community health initiatives and enhance patient care at Island Health.
