As a valued member of the inter-professional healthcare team, you will assess and facilitate complex discharge planning for clients needing community services.
Your role will involve effective communication with families and healthcare providers to ensure a seamless transition. Your skills in problem-solving and collaboration will enhance patient care outcomes.
Key Responsibilities:
• Manage complex discharge planning procedures
• Communicate with clients, families, and healthcare teams
• Evaluate and document ongoing client care needs
• Foster cooperative relationships with service providers
• Maintain expertise in community health options
Requirements:
• Graduation from an accredited nursing program
• BCCNM registered RN is mandatory
• Three years’ related experience in gerontology
• Valid Class 5 BC Driver's License required
• Strong observation and assessment skills
