Elevate your nursing career with Island Health as a Home Support CHW Supervisor, LPN. This part-time role involves supervising Community Health Workers and providing exceptional care for clients.
In this key position, you will leverage your LPN registration and three years of relevant experience to oversee quality service delivery in home support. Key Responsibilities:
• Supervise Community Health Workers in home care settings
• Implement and evaluate service delivery and care plans
• Provide LPN-standard nursing care in the community
• Conduct assessments and recognize client health changes
• Collaborate effectively with interdisciplinary teams
Requirements:
• LPN registration with BC College of Nurses and Midwives
• Minimum three years of related care experience
• One year of supervisory experience preferred
• Valid BC Driver's License required
• Physical ability to perform job duties
