JOB SUMMARY
The Community Inclusion Program Coordinator will have the personality, ability and temperament necessary to interact with the people we support, their families and advocates, Community Inclusion employees and all others in a manner that fosters rapport and encourages open communication to ensure standards are being met and quality services are provided.
The Coordinator shall be responsible for the daily operation of their assigned programs and management of their Community Inclusion employees. They are also expected to work and collaborate with colleagues as part of PCCR’s Home Share Coordinator network team.
KEY DUTIES & RESPONSIBILITIES
Responsible for attending procurements on Community Inclusion programsCreate and set up Community‑based programs for supported personsRecruitment, retention and management of Community Inclusion employeesAssist in program training, staff development and provide support and guidance to Community Inclusion employeesAssist with the development of Individualized Service Plan / Personal Service PlanSupport Home Share Providers and Community Inclusion workers in achieving goals and objectives as stated in ISP/PSPEnsure each person’s Personal Service Plan is regularly implemented, reviewed and updated focusing on meeting their needs and wantsParticipate in developing the strategic direction of Community Inclusion programs at PCCRMonitor programs and ensure all regulatory guidelines and standards including CARF are metCoordinate crisis response and assist with problem‑solving and respite if necessary; understand risk management and implement behavioral support plans as part of crisis response for Community InclusionAvailable to answer calls after hours regarding Community Inclusion concerns/issuesManage program hours accordinglyEvaluate Community Inclusion employee’s progress annually in writing; provide follow‑up direction and discipline where necessaryAttend Home Share Coordinator meetings and participate in all other required meetingsEnsure all employees are logging their hours; maintain all personnel, payroll, and financial records as requiredComplete administrative tasks related to contracting with Community Inclusion programMeet with Directors / Home Share Manager and perform tasks as requestedQUALIFICATIONS AND SKILLS
Post‑Secondary DiplomaCertificate / Diploma in Management TrainingMinimum three years experience as a Community Inclusion worker or related field or at least two years of managerial experienceAbility to develop Community Inclusion resources and manage and monitor programsExcellent oral, written and interpersonal communication skillsStrong computer skills in Microsoft Windows, including word processing, Outlook and Share VisionEvidence of practice of confidentialityCurrent First‑Aid and CPR Level C CertificationAny qualification or skill required by the funding agencyWHAT WE OFFER
