The Neighbourhood Group seeks a dedicated Mobile Shelter Support Program Case Manager to assist vulnerable populations in the GTA. This role emphasizes comprehensive case management within a trauma‑informed framework.

As part of the Urban Health and Homelessness team, you will provide essential support to individuals navigating complex issues related to health, housing, and legal services. By conducting assessments and coordinating resources, you will foster an environment of stability and empowerment for clients facing homelessness and related challenges.

Key Responsibilities

Conduct assessments and provide tailored support
Facilitate referrals and advocacy in the community
Maintain ongoing client support through home visits
Collaborate with external partners and service providers
Document case management activities and outcomes

Requirements

Bachelor’s degree and 2 years in human services
Familiar with mental health and trauma‑informed care
Commitment to harm reduction principles
Robust verbal and written communication skills
Ability to travel across the city for client support
