Position: Social Worker at Park Place Seniors Living
Location: Ladysmith

Support the well-being of seniors as a full-time Social Worker at Cerwydden Care and Oyster Harbour Care Centre. Create positive change in residents' lives throughout Vancouver Island.

This shared role entails direct engagement with residents and families, where you will assess psychosocial needs and provide essential support in care transitions. At Park Place Seniors Living, emphasis is placed on advocacy and collaboration with care teams and external resources to ensure residents receive holistic care.

Key Responsibilities:

• Perform psychosocial assessments and counselling

• Assist residents during health changes and end-of-life transitions

• Collaborate in care planning and conferences

• Inform residents of community resources and financial aid

• Champion residents’ dignity and rights

Requirements:

• Degree in Social Work (BSW or MSW)

• Certification eligibility with the BC College of Social Workers

• Experience in long-term or seniors care preferred

• Strong skills in communication and relationship building

• Valid driver’s license for site travel
