The Case Manager position is pivotal in assisting immigrants and refugees with their integration journey. Focusing on personalized case management and advocacy, you will conduct assessments and guide clients in accessing key resources.
Your role will involve collaboration and coordination among various stakeholders to ensure that client needs are met effectively.
Key Responsibilities:
• Assess client needs through comprehensive intake interviews
• Create and monitor individualized settlement plans
• Facilitate connection with community resources and services
• Act as an advocate for clients against systemic barriers
• Organize skill development and coaching opportunities
Requirements:
• Bachelor’s degree in Social Work or a relevant field
• Proven communication skills in a diverse setting
• Experience working with vulnerable groups essential
• Competency in Microsoft Office and client databases
• Required vulnerable sector criminal record check
