Make a difference in the lives of those at risk of homelessness with Island Crisis Care Society. As a Community Outreach Worker, you will provide essential outreach services and support to enhance community well-being.
This role focuses on connecting with individuals facing homelessness by collecting relevant information and devising effective intervention plans. Utilize your skills in problem-solving, active listening, and emotional support to guide clients towards stable housing. Engage actively with the integrated case management team to assess progress and ensure tailored support for each client.
Key Responsibilities:
• Identify and connect with individuals at risk of homelessness
• Gather and assess client information through interviews and observations
• Develop and implement intervention plans within guidelines
• Provide guidance and problem-solving support for housing challenges
• Collaborate with social service professionals and maintain records
Requirements:
• Degree in Social Services or equivalent experience
• Two years of relevant work experience
• Strong knowledge of substance use disorders and mental health
• Excellent interpersonal skills and problem-solving abilities
• Commitment to maintaining client confidentiality and healthy boundaries
