Connective seeks dedicated Outreach Support Workers to support clients referred by community partners. The ideal candidate will provide essential assistance in maintaining housing stability, case management, and integrated service provision. You will work closely with clients to create intervention plans tailored to their unique needs.
Key Responsibilities:
• Conduct referral consultations based on established criteria
• Collaborate with clients to assess needs and determine interventions
• Develop personalized support plans with measurable goals
• Provide case management and coordinated social support services
• Drive to various sites for client and community meetings
Requirements:
• Diploma or certificate in a relevant field with one year of experience
• Ability to respond in crisis situations effectively
• Valid Class 5 driver's license and personal transportation
• Current Emergency First Aid Certificate
• Commitment to cultural safety and reconciliation practices
