$130,000 – $160,000 per year, based on skills and experience.

Responsibilities- Providing hands‑on patient care including PICC lines, wound VACs, feeding pumps, NG/GS/J tubes, drains, Foley/suprapubic catheters, trach care, blood draws, ostomy management, injections, IV antibiotics/TPN, and compression wraps.
- Performing direct patient care as defined in the state Nurse Practice Act.
- Implementing a plan of care initiated by the Case Manager RN.
- Maintaining accurate, timely documentation within 24–48 hours.
- Assessing patients and educating families/caregivers.
- Assisting with skilled procedures and teaching self‑care techniques.
- Coordinating home‑health services and reporting needs to other disciplines.
- Using equipment and supplies efficiently.
- Pursuing personal and professional growth.
- Performing duties assigned by the Case Manager RN.
- Prioritizing care based on assigned patient communication.
- Providing timely, pertinent reports to the Case Manager.
- Recording information that reflects patient needs, challenges, capabilities, and outcomes.
- Documenting incidents accurately.
- Reporting information promptly to the Case Management RN and providers.
- Attending weekly Case Conferences as required.
- Ensuring patient privacy and maintaining medical records confidentiality.
- Maintaining professionalism and enhancing clinical knowledge.
- Meeting productivity standards directed by the Case Manager RN.
- Reviewing Start‑of‑Care assessments with the Case Manager.
- Participating in on‑call duties during weekend rotations.
- One (1) year of recent nursing experience (waivable for internal candidates and new grads).
- Preferred:
One year of home‑health experience. - Experience with high‑acuity patients is a plus.
- Experience working with interdisciplinary teams.
- Bachelor's degree from an accredited nursing program.
- California Registered Nurse (RN) license.
- Basic Life Support (BLS) certification.
- Valid California driver's license with an acceptable driving record.
- All licenses and certifications current at hire and sustained throughout employment.
- Self‑directed, critical thinker, outstanding interpersonal communication, organized, computer literate, observant, and proficient in basic math and nursing competencies.
