Hospice Registered Nurse (RN)

- Provide care and services for a group of patients, including home visits, skilled nursing facilities, and assisted living facilities.
- Work as a contributing member of the interdisciplinary team, offering expertise in psychosocial assessments and community resources.
- Serve in either a Case Management, Admissions, or Resource role.
- Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Work independently with day‑to‑day autonomy, managing phone, email, and written communication, and attending meetings with various healthcare professionals.
- Manage visits, documentation, and meetings, prioritizing and triaging multiple concerns.
- Learn and use an electronic medical records system.
- Be willing to drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area.
- Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
- 1 year of hospice experience.
- Strong teamwork skills and ability to collaborate daily with a multidisciplinary team.
- Excellent time management and organizational skills.
- Ability to learn an electronic medical records system.
- Valid driver’s license and reliable transportation for travel across the service area.
