Weekend Admission Nurse; RN

Part Time Registered Nurse Case Manager

Anticipated Territory:
Napa Area.

Schedule:

Must be available full days Saturday and Sunday (4 SOCs/day). Also open to full time:
Saturday, Sunday plus 3 weekdays.

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow-ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Initiate and administer IV medication infusions safely and effectively in accordance with established protocols
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided

• Concierge for Case Managers:
  Dedicated resources assist with time consuming administrative tasks
• Flexible work schedules:
  Create your own schedule and hours

  Typically patient visits are within a 5-15 mile radius

• Current and unrestricted CA RN license
• Current BLS/CPR Certification
• Valid CA Driver's License
• Experience conducting assessments and Start of Care (SOC) - OASIS
• High level of clinical knowledge in home health care
• Experience with high acuity patients a plus
• Experience working with an interdisciplinary team
• Case Management fundamentals
• Previous experience with IV Medication infusions
• Proficiency initiating intravenous (IV) therapy
