Millennium Home Care (an affiliate of Millennium Healthcare, LLC) is seeking dedicated PRN Speech–Language Pathologist (SLP) to join a leading in‑home healthcare provider. Responsibilities- Conduct comprehensive evaluations of patients’ speech, language, cognitive‑communication, and swallowing abilities to identify strengths, weaknesses, and treatment needs.
- Develop individualized treatment plans addressing specific goals in speech sound production, language development, fluency, voice disorders, and swallowing difficulties.
- Deliver direct therapy services such as articulation, language, fluency, voice, and swallowing therapy using evidence‑based practices.
- Educate patients and families on strategies to improve communication skills, swallowing safety, and overall quality of life.
- Collaborate with physicians, occupational therapists, and physical therapists to create comprehensive care plans and ensure optimal patient outcomes.
- Maintain accurate and timely documentation of evaluations, treatment plans, progress notes, and discharge summaries.
- Adhere to all regulatory standards and guidelines, including HIPAA and Medicare regulations.
- Current and valid Speech‑Language Pathologist (SLP) license in the state of Florida.
- Minimum of 1 year of experience in a hospital, rehabilitation center, or skilled nursing facility.
- Strong clinical skills, excellent communication and interpersonal abilities, and a patient‑centered approach to care.
- Strong work ethic, reliability, and commitment to providing high‑quality care.
- Reliable transportation to visit patients in their homes.
