Job Summary: The Physical Therapist evaluates, plans, and administers therapeutic procedures in physical medicine and rehabilitation as prescribed by a referring physician. This role establishes individualized physical therapy goals, provides treatments, and regularly reassesses patient progress, documenting all findings accurately.

Essential Functions
  • Conduct thorough physical therapy evaluations to assess patient needs and develop individualized treatment plans.
  • Administer safe and effective therapy treatments, ensuring all interventions are appropriately documented.
  • Reassess and adjust treatment plans within appropriate time frames to align with patient progress and changing needs.
  • Provide clear, concise, and accurate documentation of evaluations, treatment plans, progress notes, and billing information.
  • Educate patients and their families on treatment plans, therapeutic exercises, and post‑treatment care to support long‑term recovery.
  • Operate therapy equipment safely and maintain a clean, organized, and safe environment for patients and staff.
  • Collaborate with referring physicians, peers, and interdisciplinary team members to ensure continuity and quality of patient care.
  • Demonstrate population‑appropriate competencies for all relevant patient demographics, tailoring care to individual needs.
  • Maintain compliance with facility policies, including safety, infection control, and risk management procedures.
  • Adapt to changes professionally and effectively, following the chain of command to address and resolve issues impacting patient care.
  • Perform other duties as assigned.
  • Maintain regular and reliable attendance.
  • Comply with all policies and standards.
Qualifications
  • Doctorate in Physical Therapy (DPT) required.
  • 0–2 years of experience in a physical therapy setting required.
Knowledge,

Skills and Abilities
  • Strong knowledge of physical therapy principles, therapeutic techniques, and safety standards.
  • Ability to perform comprehensive evaluations and develop effective treatment plans.
  • Proficiency in using therapy equipment and technology for documentation and billing.
  • Excellent communication and interpersonal skills to collaborate with patients, families, and interdisciplinary teams.
  • Strong organizational and time‑management skills to prioritize tasks and maximize productivity.
  • Commitment to professional development and adherence to ethical and legal standards.
Licenses and Certifications
  • PT – Physical Therapist license in the state of employment required.
  • BCLS – Basic Life Support within 30 days of hire required.
