Moorings Park is seeking a dedicated and talented Occupational Therapist to join our Certified Home Health team. The Occupational Therapist is responsible for ensuring and coordinating quality and safe delivery of home health occupational therapy services following applicable licensure/certification requirements both State and Federal. The Occupational Therapist has responsibility for the planning and therapeutic interventions of all occupational therapy cases. Occupational Therapy will be provided by a licensed Occupational Therapist or a licensed Occupational Therapy Assistant under the supervision of a licensed Occupational Therapist in compliance with the Plan of Care.

• Evaluates the patient’s needs, including medical, psychosocial and environmental needs.
• Develops a clinical assessment based on evaluation/re-evaluation findings and requisition and management of equipment and supplies required to increase the clients function and independence as appropriate to care.
• Develop and implements, with physician and patient, individualized Plan of Care based on patient’s physical, medical, psychosocial and environmental needs, and makes appropriate revisions as the patient’s condition changes.
• Establishes appropriate realistic and measurable goals for patients to achieve, then evaluates progress towards these goals.
• Obtains initial and interim physician's orders and reports on progress and changes in client condition to physician and Clinical Manager.
