• Serve as the primary case manager for assigned patients, coordinating all aspects of care and ensuring compliance with facility policies.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Communicate regularly with physicians, nurses, and support staff on patient status and progress.

• Registered Nurse (RN) license, active in Florida.
• Minimum 2+ years of clinical experience in your specialty.
• BLS certification from the American Heart Association.
