The RN Navigator Vascular Surgery works independently to plan, implement, and evaluate nursing care in patient care situations using standards of care, policy, and procedure, in coordination with the Vascular Surgery department and the Vascular Surgery Team. Provides direct nursing care and assumes teaching and leadership responsibilities for Vascular Surgery patients, participates in shared decision making, referrals, pre- and post-surgery.

• Work directly with the Vascular Surgeons/ PAs to provide care to Vascular Surgery patients.
• Coordinate the referral process and introduction of patients into the Vascular Surgery Program.
• Employ patient teaching to promote health and safety; evaluate progress toward outcomes; practice ethically.
• Receive incoming referrals from physicians and hospitals and respond quickly to ensure rapid access into the system.
• Triages referrals according to written criteria to assure patient needs are met on a priority level.
• Communicate plans for acceptance into the program to referring physicians/hospitals and coordinate patients to tertiary care centers as needed.
• Educate patients and families regarding the referral process, evaluation steps, and keep them informed throughout the process.
• Demonstrate leadership in the professional practice setting and collaborate with the inter-professional team.
• Utilize resources to provide safe, effective, and financially responsible nursing care; practice in an environmentally safe manner.
• Outpatient responsibilities for nursing care of patients and assist with inpatient education in conjunction with Vascular Surgery post-op teaching.
• Collaborate with providers and clinic staff to ensure smooth transitions during preadmission, hospitalization, discharge, and outpatient services.
• Develop office protocol for referrals and workups for consults; perform thorough patient assessments; schedule and manage testing per protocol; review with Vascular Surgery team.
• Maintain documentation and databases related to Vascular Surgery; participate in outreach to referring physicians; oversee staff education and competencies as needed.
• Coordinate post-discharge clinic visits to evaluate progress and address care issues; work with Home Health Agencies for post-operative needs.
• Participate in research projects related to the Vascular Surgery Program and stay current with related literature and standards.
• Provide written reports to support research and share evidence-based practices with the office team.

• BSN preferred.
• Minimum of 2 years of experience in cardiac care.
• Licensed Registered Nurse (RN) in the State of Florida.
• Basic Life Support (BLS) certification from American Red Cross or American Heart Association.
• Strong communication and problem-solving skills with adaptability.
• Intermediate computer knowledge (MS Word, Excel, Outlook, Windows).
