This is a PRN position. Medicare Home Health experience preferred.

Job Summary

The RN utilizes the nursing process (assessment, planning, intervention, implementation, evaluation) to manage pain and other complex symptoms, provide education and support to patients and families, and help clients achieve optimal functional levels.

Job Duties

• Initial and ongoing comprehensive assessment of the client’s needs, including OASIS assessments.
• Initiate and revise the plan of care as necessary.
• Provide specialized nursing services requiring substantial skill.
• Counsel clients and families to meet nursing and related needs.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes.
• Coordinate services and referral to other providers.
• Inform physicians and staff of changes in client needs.
• Evaluate outcomes of care.
• Supervise LPNs/LVNs and home health aides.
• Participate in in‑service programs.
• Plan for discharge from service.
• Implement the Home Health Aide care plan and conduct supervisory visits.
• Teach families and caregivers.
• Participate in bi‑weekly case conferences.
• Participate in QA programs as needed.
• Follow HIPAA guidelines at all times.
• All other duties as assigned.

Education / Experience

Graduate of an accredited nursing school; one year of medical/surgical RN experience.

Certificates, Licenses, Registrations

• Valid Florida Driver’s License.
• Current Florida Registered Nurse license.
• Healthcare Provider CPR required upon hire and maintained.

Supervisory Responsibilities

May supervise LPNs and home health aides as appropriate.

Computer Skills

Basic typing, data entry, and word processing; database software; general knowledge of e‑mail.

Language Skills

Read, analyze, and interpret common scientific and technical journals; respond to inquiries or complaints from customers and regulatory agencies.

Mathematical Skills

Work with data, probability, statistics, fractions, percentages, ratios, and proportions.

Reasoning Ability

Define problems, collect data, establish facts, and draw valid conclusions; interpret varied instructions, diagrams, and variables.

Physical Demands

Regular sit, hand use, talk or hear; frequent stand, walk, and reach; occasional stoop, kneel, crouch, or crawl; lift up to 50 lbs frequently; specific vision requirements.

Work Environment

Occasional exposure to outside weather conditions; moderate noise levels.
