• Work in collaboration with the RN to fulfill patient‑specific care plans.
• Collaborate with the care team to ensure all patient needs are fully addressed.
• Deliver high‑quality skilled care to patients.
• Thoroughly document care delivery daily in the EMR system.
• Contribute to a culture of caring through accountability and teamwork.

• Current Licensed Vocational/Practical Nurse license.
• Valid state driver’s license and auto liability insurance.
• One year experience as a Licensed Vocational/Practical Nurse in a clinical care setting; home health experience preferred.

• Compassionate care delivery with a focus on results.
• Solution‑driven, self‑motivated, able to respond with urgency.
• Love learning and motivating patients to reach goals.
• Able to work independently without direct oversight.
• Able to discern when to call for support and communicate challenges.
• Comfortable with technology; HCHB experience is a plus.
