Ambulatory RN: Telehealth Care & Mentorship

Southern New Hampshire Health is seeking a Registered Nurse Ambulatory to provide professional nursing care across Foundation practices, in-person and via telephone triage and telehealth. You will work with providers and care teams to deliver safe, evidence-based, patient-centered care.

Responsibilities include conducting the nursing process in ambulatory and telehealth settings, collaborating on care plans, administering medications and vaccines, and educating patients on self-management.
