Organization:

Pharmacy Services

Reviews medication orders issued by physicians or other authorized prescriber to assure accuracy and determine formulas and ingredients needed, assuring that all orders are filled and labeled (including secondary labels) properly. Performs clinical functions as outlined by the Pharmacist in Charge. Maintains the integrity and accuracy of the drug distribution and delivery systems. Ensures the appropriate and safe utilization of high‑risk medications and controlled substances as outlined by department and hospital policies.

Maintains patient profiles with current information on allergies, diagnosis, weight, current medication and IV therapy orders. Compounds and supervises the compounding of medications and intravenous solutions including hazardous medications using standard formulas and processes. Review patient's drug regimen for potential interactions, interferences or incompatibilities and notify physician or nursing staff as appropriate. Assist in training pharmacy technicians and student pharmacists.

  • Self‑motivated. Fulfills areas of responsibility without prompting.
  • Supervises pharmacy technicians and students giving constructive feedback in real time.
Technical Capabilities
  • Medication Management (Intermediate):
    Demonstrates proficiency in medication management in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully conducting Medication Management without requiring support and instruction from others. Has the ability to determine the clinical appropriateness of each medication taken by the patient, determine the appropriateness of the dose, dosage form, indication, contraindications, potential adverse effects and potential problems with concomitant medications.

    Able to monitor for therapeutic duplication or other unnecessary medications, interpret, monitor and assess patient’s laboratory results as they relate to medication therapy and provide drug information to other healthcare professionals, patients and their families.
  • Pharmaceutical Drug Preparation & Dispensing (Intermediate):
    Demonstrates proficiency in drug preparation and dispensing including practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to review, prepare and dispense prescribed medications. This includes being proficient in the general principles of aseptic technique utilized to compound sterile preparations (CSPs) to minimize the risk of contamination.
