Patient Care Coordinator – Results Physiotherapy

Job Description

The Patient Care Coordinator - I (PCC-I) supports clinic growth through excellence in execution of the practice management role and patient intake processes. This individual works in collaboration with the Clinic Director (CD) to carry out efficient clinic procedures and supports the mission, vision, and values of Upstream Rehabilitation.

Responsibilities
  • Collect all money due at the time of service.
  • Convert referrals into evaluations and schedule patient visits.
  • Provide excellent customer service: greet patients, make welcome calls, monitor arrival rate, and create a welcoming clinic atmosphere.
  • Manage the daily practice schedule, document routing, personal overtime, non‑clinical documentation, deposits, caseload, discharge candidates, progress notes, and insurance reporting.
  • Monitor clinic inventory and oversee logistical needs.
  • Attend required training with Territory Field Trainers for Raintree and other business process updates.
  • Complete quarterly compliance training.

Position Summary

Qualifications
  • High School Diploma or equivalent.
  • Excellent communication skills and ability to relate well to Business Office and Field leadership.
  • Multitasking ability with strong organizational detail, meeting deadlines, and working independently.
  • Efficient time management and presentation skills as a team member.
Physical Requirements
  • Full‑time position during normal business hours Monday through Friday, with a minimum of 40 hours per week. Occasional evening or weekend work may be required.
  • Primarily sedentary work with frequent use of a computer, phone, and other office equipment.
  • Ability to sit, stand, and use repetitive motions for typing and communicating with colleagues.
  • Capable of moving, pushing, pulling, or carrying up to 20 lbs; seldom required to lift under 10 lbs.
  • Must be able to view a computer screen for extended periods and comprehend information to respond appropriately.
