Overview
Elara Caring is seeking a Speech Language Pathologist to join our home-based care teams. We provide high-quality care in patients’ homes and aim to deliver Right Care, at the Right Time, in the Right Place.
Responsibilities- Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care.
- Evaluates patients to determine rehabilitation needs and develops a plan of care.
- Admits patients and assesses speech-language therapy and other needs according to OASIS and PPS; conducts initial and follow-up evaluations as required.
- Initiates plan for patient safety and uses patient, family, and community resources.
- Communicates findings and changes to supervisor, physician, and team members involved in patient care.
- Participates in Plan of Care development and discharge planning; verifies Plan of Care prior to visits and provides care per physician orders and standards.
- Writes physician orders for additional visits or changes to the Plan of Care as needed.
- Incorporates patient care goals into therapy care and revises the Plan of Care as issues arise.
- Performs direct Speech-Language pathology services per accepted standards and physician certification.
- Selects diagnostic and therapeutic materials and instructs patients and caregivers in the home therapy program.
- Ensures notes reflect ongoing coordination of services with physician and other staff; maintains accurate therapy and visit logs and submits according to policy.
- Adheres to a physically demanding, potentially high-stress environment and handles patient transfers.
- May require irregular hours including call hours if applicable.
- Master’s degree in Language Pathology from an accredited program.
- Current, unrestricted SLP license in the practicing state.
- Minimum one year of clinical SLP experience.
- Able and willing to travel within branch/office coverage area; must have dependable vehicle, valid driver’s license, and current auto insurance.
- Willing to travel up to 50% of time.
- Ability to sit, stand, bend, lift and move intermittently; ability to lift 50-100 lbs.
You will report to the Branch Director, Clinical Manager, Clinical Supervisor or designee.
