* Candidates must have related or direct experience working with the community we serve.*

• Promotes optimal health for individuals through routine, preventive, and emergency health care.
• Assists with all hands-on direct care, including showering, toileting, shaving, brushing teeth, and meal-time assistance.
• Ensures that residential environments are safe and secure and where individuals are free from harm.
• Provides education, training, counseling, and physical assistance when necessary to achieve ISP goals in areas such as functional academics, domestic skills, health and personal hygiene, social and leisure skills, communication skills, community mobility, self-esteem, and money management.
• Develops, writes, and presents annual ISPs and semi-annual assessments in consultation with other residential staff and directors.
• Provides transportation to individuals as necessary.
• Communicates with family, guardians, and other relevant parties to facilitate consistency of programming and support.
• Performs case management duties including maintenance of confidential files, documentation of activities, and attending regular meetings as necessary.

• Experience working with individuals with /DD
• High school Diploma/GED; experience is preferred in a related field
• Must be able to pass Fingerprint background check, CORI, DPPC and driving background checks
• Valid Driver's License required
• Valid MAP certification preferred
• Excellent verbal and written communication skills
