• Advocate for the well-being of residents and ensure their holistic needs are met, enhancing quality of life.
• Conduct comprehensive psychosocial assessments to identify residents’ social, emotional, and environmental needs, strengths, and preferences.
• Collaborate with residents, families, and the interdisciplinary team to develop individualized care plans addressing social, emotional, and psychological concerns.
• Provide counseling and emotional support to residents and families, offering guidance on coping strategies, adjustment to care transitions, and decision-making processes.
• Advocate for residents’ rights and preferences, ensuring their voices are heard in care planning and decision-making.
• Connect residents and families with community resources, support services, and advocacy organizations to address social, financial, and legal needs.
• Provide crisis intervention and support during challenging situations, such as grief, family conflicts, or changes in health status.

• Master’s degree in social work from an accredited program
• Current licensure or certification as a Social Worker in the state of employment
• Previous experience in long-term care, geriatrics, or a related field
• Strong assessment, counseling, and crisis intervention skills
• Excellent communication and interpersonal abilities
• Ability to work collaboratively as part of a multidisciplinary team
• Empathy, compassion, and a commitment to person-centered care
