What You’ll Do- Advocate for the well-being of residents and ensure their holistic needs are met, enhancing quality of life.
- Conduct comprehensive psychosocial assessments to identify residents’ social, emotional, and environmental needs, strengths, and preferences.
- Collaborate with residents, families, and the interdisciplinary team to develop individualized care plans addressing social, emotional, and psychological concerns.
- Provide counseling and emotional support to residents and families, offering guidance on coping strategies, adjustment to care transitions, and decision-making processes.
- Advocate for residents’ rights and preferences, ensuring their voices are heard in care planning and decision-making.
- Connect residents and families with community resources, support services, and advocacy organizations to address social, financial, and legal needs.
- Provide crisis intervention and support during challenging situations, such as grief, family conflicts, or changes in health status.
- Master’s degree in social work from an accredited program
- Current licensure or certification as a Social Worker in the state of employment
- Previous experience in long-term care, geriatrics, or a related field
- Strong assessment, counseling, and crisis intervention skills
- Excellent communication and interpersonal abilities
- Ability to work collaboratively as part of a multidisciplinary team
