Job Summary

Performs moderately complex case management work. Work involves developing and maintaining long-term contact with inmates, inmate families, and service providers for medical, social, educational, and related service needs. Works under general supervision with moderate latitude for the use of initiative and independent judgment.

Essential Functions
  • Provides ongoing case management and serves as a liaison with inmates, inmate families, and service providers; implements follow-up and aftercare plans for inmates who participate in the substance use treatment programs; and disseminates information on community resources for inmate placement in follow-up treatment.
  • Interviews inmates to gather information and assess service needs, problem areas, service gaps, and areas that may be barriers to services; develops and implements service and treatment plans to meet inmate needs; coordinates, participates in, and assists in conducting educational, skill development, and group activities in accordance with substance use treatment program requirements, curriculum and agency standards; and participates in treatment team meetings.
  • Coordinates activities within the treatment program; carries a caseload for cognitive-based services; works with the treatment team of licensed chemical dependency counselors, qualified credentialed counselors, security staff, supervision officers, and others to monitor inmate progress; and makes recommendations in treatment planning.
  • Maintains inmate case files and other records regarding services provided to include workload statistics; prepares lesson plans; and assists in the preparation and presentation of educational modules.
  • Conducts didactical group sessions; assists in preparing and conducting inmate orientation and intake sessions; and monitors the operation of peer support and twelve step recovery groups.
  • Performs a variety of marginal duties not listed, to be determined and assigned as needed.
Minimum Qualifications Education, Experience, and Training
  • Graduation from an accredited senior high school or equivalent or GED.
  • One year full‑time, wage‑earning experience in customer service, clerical, secretarial, criminal justice, correctional custody, law enforcement, case management, technical program support, operational support, social services, case processing, rehabilitative programming, teaching, or counseling. Fifteen semester hours from a college or university accredited by CHEA or USDE may be substituted for each six months of experience.
  • Rehabilitation programming experience preferred.
  • Transitional and reentry planning experience preferred.
Knowledge and Skills
  • Knowledge of case management principles, objectives, standards, and methods.
  • Knowledge of applicable state and federal laws, rules, regulations, and statutes.
  • Knowledge of agency and departmental organizational structure, policies, procedures, rules, and regulations preferred.
  • Knowledge of agency substance abuse treatment program policies and procedures preferred.
  • Skill to communicate ideas and instructions clearly and concisely.
  • Skill to coordinate with other staff, departments, officials, agencies, organizations, and the public.
  • Skill to interpret and apply rules, regulations, policies, and procedures.
  • Skill in problem‑solving techniques.
  • Skill to prepare and maintain accurate records, files, and reports.
  • Skill to review technical data and prepare technical reports.
  • Skill to plan work in order to meet established guidelines.
  • Skill to assess inmate needs, coordinate inmate services, and implement program services.
