As a Physical Therapist at our facility, you will evaluate, plan, and deliver evidence-based physical therapy services to residents in accordance with physician orders, state practice acts, and federal regulations. You’ll have the opportunity to make a powerful impact on each resident’s functional abilities, quality of life, and overall well-being.

• Conduct comprehensive evaluations and develop individualized treatment plans

• Provide therapeutic interventions to improve strength, mobility, balance, and functional independence

• Document assessments, progress notes, and therapy minutes in compliance with facility standards and Medicare/Medicaid guidelines

• Coordinate care with an interdisciplinary team of nurses, therapists, physicians, and social services

• Participate in care plan meetings to support effective discharge planning

• Educate residents and families on exercises, safety, and strategies for continued success

• Ensure therapy equipment is used safely, maintained, and properly sanitized

• Assist with monitoring department performance indicators and maintaining regulatory readiness

• Promote a positive, compassionate, resident-centered care environment

• Active, unrestricted Physical Therapist license in the state

• Graduate of an accredited Physical Therapy program

• Current CPR certification (or willingness to obtain)

• Valid driver’s license in the state of employment

• Knowledge of skilled nursing facility operations and long-term care regulations preferred

• Strong communication, documentation, and clinical reasoning skills
