About the RoleAs a Physical Therapist at our facility, you will evaluate, plan, and deliver evidence-based physical therapy services to residents in accordance with physician orders, state practice acts, and federal regulations. You’ll have the opportunity to make a powerful impact on each resident’s functional abilities, quality of life, and overall well-being.
What You’ll DoConduct comprehensive evaluations and develop individualized treatment plans
Provide therapeutic interventions to improve strength, mobility, balance, and functional independence
Document assessments, progress notes, and therapy minutes in compliance with facility standards and Medicare/Medicaid guidelines
Coordinate care with an interdisciplinary team of nurses, therapists, physicians, and social services
Participate in care plan meetings to support effective discharge planning
Educate residents and families on exercises, safety, and strategies for continued success
Ensure therapy equipment is used safely, maintained, and properly sanitized
Assist with monitoring department performance indicators and maintaining regulatory readiness
Promote a positive, compassionate, resident-centered care environment
Active, unrestricted Physical Therapist license in the state
Graduate of an accredited Physical Therapy program
Current CPR certification (or willingness to obtain)
Valid driver’s license in the state of employment
Knowledge of skilled nursing facility operations and long-term care regulations preferred
Strong communication, documentation, and clinical reasoning skills
