APRN or PA-C at Phelps Medical Group (Healthcare)

At Phelps Medical Group, we are looking for a Nurse Practitioner or Physician Assistant that has experience in Womens Health and Primary Care.

SUMMARY

The Advanced Practice Registered Nurse (APRN) or Physician Assistant Certified (PA-C) is responsible for providing medical and/or emergency services to support the efficient operation of a rural health clinic (RHC). They emphasize womens health, primary health care, disease prevention, patient education, and continuity of care as stated in their scope and privileges of practice in their credentialing file. They ensure high‑quality and compassionate care to patients across all ages and backgrounds in compliance with rules and regulations of all applicable federal, state and local laws as well as Phelps Medical Group and Phelps Memorial Health Center policies is a condition of employment.

ESSENTIAL DUTIES AND RESPONSIBILITIES 1. Patient Care
  • Provides services within accepted professional healthcare standards of care in accordance with safe and accepted patient care practices and current ethical standards.
  • Practices healthcare and renders professional healthcare services in the execution of duties that include, but are not limited to, general acute patient care, subacute patient care, emergency patient care, and primary care.
  • Performs a history and physical for complex acute, critical, and chronically ill patients.
  • Orders and interprets diagnostic and therapeutic tests relative to patients age‑specific needs.
  • Prescribes appropriate pharmacologic and non‑pharmacologic treatment modalities.
  • Monitors the effectiveness of interventions.
  • Facilitates the patients transition within and between healthcare settings, e.g., admitting, transferring, and discharging patients.
  • Performs hospital rounds, documents detailed narrative and case summaries, completes forms pertinent to patients medical records, issues diagnostic orders with timely signatures in accordance with clinic and hospital guidelines and payer requirements.
  • Provides educational materials to staff and patients.
  • Participates in the RHCs and hospitals quality assurance program and provides all services in accordance with applicable licensure, accreditation and reimbursement standards and Medicare conditions of participation.
  • Follows the Medical Staff Bylaws, rules and regulations and the administrative policies of the RHC and hospital.
  • Performs other duties as assigned.
2. Administrative and Communication
  • Collaborates with multidisciplinary team members by making appropriate referrals.
  • Assists in educational, training, and in‑service programs.
  • Participates, on a reasonable and equitable basis with other providers, in the affairs of committees established by the organization.
  • May be assigned as a committee member for the RHC Review Committee.
  • Participates in a chart review with the Clinic Medical Director.
  • Maintains accurate, timely, and complete documentation in the electronic health record (EMR) in accordance with the Medical Staff Bylaws.
  • Participates in Medical Staff Meetings, RHC provider meetings, in‑service training and quality improvement activities.
  • Ensures patient confidentiality and compliance with HIPAA regulations.
  • Works collaboratively with physicians, nurses, medical assistants, and administrative staff.
3. Emergency and Safety Responsibilities
  • Recognizes and responds to emergency situations (e.g., patient distress, falls, or injury) per clinic protocol.
  • Follows all infection control, safety, and hazardous material procedures.
4. APRNs assigned to the Emergency Department
  • Assesses and manages patients presenting to the emergency department during his/her shift and will communicate with the on‑call physician for the Emergency Department as necessary.
  • Consults with on‑call physician as outlined in Mid‑Level Provider (PA-C/ Nurse Practitioner) Management of Emergency Situations.
  • Responds to Rapid Response and Code Team activations as available.
  • Supervises cardiac stress testing.
