Hospice Social Worker: Family & Crisis

Angels Care Hospice in the United States seeks a social worker to implement standards of care for medical social work services, assess psychosocial status of patients with terminal illness, and plan safe, appropriate support within the patient’s home or care environment.

You will educate interdisciplinary teams, maintain clinical records, serve as liaison with families and community agencies, and participate in quality improvement activities;
