We are seeking a friendly and compassionate Primary Care Provider to join our team. As a Primary Care Provider you will play a crucial role in providing comprehensive care to patients in long-term care facilities. This is an exciting opportunity to make a difference in the lives of patients while working alongside a dedicated team of healthcare professionals.

Responsibilities
  • Provide comprehensive healthcare services and coordinate patient care
  • Work closely with a team of healthcare professionals
  • Assess, diagnose, and treat residents  physical and mental well-being
  • Conduct thorough patient assessments, including medical history reviews and physical examinations
  • Order and interpret diagnostic tests
  • Develop accurate diagnoses and treatment plans
  • Prescribe medications and administer treatments
  • Provide counseling to residents and their families
  • Collaborate with the interdisciplinary team for seamless coordination of care
  • Manage chronic conditions and monitor progress
  • Make appropriate adjustments to care plans
  • Commitment to person-centered care
  • Create a safe and nurturing environment for elderly residents in the nursing home
Requirements
  • Nurse Practitioner or Physician Assistant License
  • DEA License
  • 6 months experience in long-term care
  • Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day
  • Malpractice insurance coverage
