• School Day Shift: 8a 1130a
• Client Specifics: 3 year old with a g tube HCN would attend school with client

• Provide skilled nursing care in home and school settings
• Assess and monitor your patients condition and communicate changes with the care team
• Administer medications treatments and prescribed therapies
• Provide trach ventilator airway and G tube care as needed
• Maintain accurate documentation and support families through education
• Follow individualized care plans and maintain a safe care environment
