Angels Care Hospice is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate all aspects of the patient’s home care. You will work with other medical disciplines and provide professional care in the patient’s home.
You will perform the admission visit, determine eligibility for home care services, and develop the plan of care to be followed. A valid RN license, current driver’s license, and reliable transportation are required.
