Kenniscentrum Sociaal Domein zoekt een Sociaal Werker Wmo om Wmo-onderzoeken uit te voeren en inwoners te begeleiden bij complexe ondersteuningsvragen. Je onderzoekt hulpvragen, voorzieningen en netwerk, en onderbouwt je bevindingen vanuit het juridische kader.
Je werkt toekomstgericht, stelt rapportages op en draagt bij aan een goede teamdynamiek. Een relevante HBO-opleiding en minimaal 5 jaar Wmo-ervaring zijn vereist.
