Location: Nelson

We are looking for a motivated and compassionate Health and Wellbeing Coach to join our Pendle West Primary Care Network (PCN) team. You will work with patients to support them in improving their health and wellbeing through personalised, non-judgemental coaching approaches.

You will manage a caseload of patients, helping them to identify goals, build confidence, and develop the skills needed to better manage their health and live independently. You will work closely with GPs, the multidisciplinary team and social prescribing services to ensure coordinated, holistic care.

The role will a fixed 2 year part time post. There will be an expectation to hold face-2-face clinics at various GP sites in Nelson and Brierfield.

Main duties of the job

Deliver one-to-one health coaching using a person-centered approach

Support patients to set and achieve personalised health and wellbeing goals

Identify patients who would benefit from health coaching via MDT and IT systems

Work in partnership with social prescribing services and community organisations

Encourage self-management, behaviour change and shared decision-making

Maintain accurate records and manage risk appropriately

Pendle West PCN is made up of seven GP practices within Nelson and Brierfield and covers a local population of circa 50,000. Job responsibilities

Manage and prioritise a caseload, in accordance with the health and wellbeing needs of thelocal PCN population through taking an approach that is non-judgemental, basedon strong communication and negotiation skills, while considering the wholeperson when addressing existing issues.

Where required and as appropriate, the Health and Wellbeing Coach will refer peopleback to other health professionals within the PCN.

Utiliseexisting IT and MDT channels to screen patients, with an aim to identify thosethat would benefit most from health coaching.

Provide personalised support to individuals, their families, and carers to support themto be active participants in their own healthcare, empowering them to manage their own health and wellbeing and live independently through -

coaching and motivating patients through multiple sessions to identify their needs, setgoals, and supporting patients to achieve their personalised health and careplan objectives.

providinginterventions such as self-management education and peer support.

supporting patients to establish and attain goals that are important to the patient, supporting personal choice and positive risk taking while ensuring that patients understand the accountability of their own actions and decisions, thusencouraging the proactive prevention of further illnesses.

Work inpartnership with the social prescribing service to connect patients to community-based activities which support them to take increased control oftheir health and wellbeing, and increasing patient motivation to self-manageand adopt healthy behaviours.

Work inpartnership with patients to support them to develop their level of knowledge,skills and confidence enabling them to engage with their health and well-beingand subsequently supporting them in shared decision-making conversations.

Utilisehealth coaching skills to support to develop the knowledge, skills, and confidence of the patient to manage their health and wellbeing, whilstincreasing their ability to access and utilise community support offers.

Explore and support patient access to a personal health budget, where appropriate, fortheir care and support.

Primary Duties and Areas of Responsibility

Developcollaborative relationships and work in partnership with health, social care,and community and voluntary sector providers and multi-disciplinary teams toholistically support patients wider health and well-being, public health, and contribute to the reduction of local health inequalities.

Provide education and specialist expertise to PCN staff, supporting them to improve their skills and understanding of personalised care, behavioural approaches and ensuring consistency in the follow up of peoples goals with MDT input; and raise awareness within the PCN of shared decision-making and decision support tools.
