Sensire zoekt een Pionier Buurtverkenner voor de regio Aalten en Dinxperlo. In deze rol ben je de schakel tussen professionele zorg en de lokale gemeenschap, waarbij je ondersteuning biedt aan cliënten om zelfredzaam thuis te blijven wonen.
Je werkt samen met de wijkverpleegkundige en bent verantwoordelijk voor welzijn en sociale basis. De functie biedt veel vrijheid, met een contract van 16 uur per week voor 7 maanden, ingeschaald in FWG 55, inclusief eindejaarsuitkering en vakantiegeld.
