Stichting Zozijn Zorg zoekt een betrokken Begeleider voor 16–24 uur per week op Kemphaan 7 in Raalte Noord. Je ondersteunt bewoners met een persoonlijke zorg- en ondersteuningsplan en bouwt een vertrouwensband op met cliënten.
Je signaleert veranderingen in gedrag en afstemming met de coördinerend ondersteuner. Een mbo-niveau 3 zorg- of welzijnsopleiding is vereist, interesse in gehandicaptenzorg is een pré.
