Zorgplein Noord zoekt een Sociaal Psychiatrisch Verpleegkundige (SPV) met BIG‑registratie voor outreachende zorg in Noordoost‑Groningen, locatie Hoogezand. Je voert intakegesprekken, formuleert hulpvragen, stelt behandelplannen op en bespreekt deze in het multidisciplinaire team.
Je werkt ambulant en bezoekt cliënten thuis. Je handelt volgens het FACT‑model, met aandacht voor herstel, regie en samenwerking met het netwerk.
