Driestroom zoekt een persoonlijk begeleider voor de locatie Van Gelrestraat in Huissen. Je begeleidt bewoners met een verstandelijke beperking, autisme en normale begaafdheid, en helpt hen talenten te ontdekken en te benutten.
Je stelt zorgplannen op, biedt maatgerichte begeleiding en werkt nauw samen met collega’s, familie en betrokkenen. Ervaring binnen begeleid wonen is gewenst; relevante mbo-4 diploma is vereist.
