Fivoor zoekt een maatschappelijk werker voor de Forensisch Psychiatrische Afdeling (FPA) Utrecht in Den Dolder. Je werkt in een hecht multidisciplinair team en levert input voor evaluaties, vertaalt behandeldoelen naar concrete plannen en ondersteunt patiënten bij huisvesting, financiën en nazorg.
Je werkt binnen een sociaal-therapeutisch behandelmilieu aan herstel en maatschappelijke terugkeer, met focus op samenwerking met verwijzers en naasten.
