Alliade zoekt een enthousiaste persoonlijk begeleider voor een woonlocatie met 24-uurs ondersteuning in Damwâld. Je geeft professionele begeleiding aan cliënten met LVB/MVB, van 23 tot 81 jaar, en werkt samen met een hecht team aan structuur en dagelijkse ondersteuning.
Je taken omvatten het opstellen en bewaken van ondersteuningsplannen, begeleiden van cliënten, functionele aanwijzingen aan collega's en het deelnemen aan teambesprekingen.
