Emergis zoekt een maatschappelijk werker voor de acute ggz, werkzaam op de medium care (MC) en high & intensive care (HIC) afdelingen. Je biedt intensieve, multidisciplinaire zorg aan volwassenen in een crisissituatie, gericht op herstel, veiligheid en het voorkomen van dwang.
Je analyseert hulpvragen, stelt behandelplannen op en werkt nauw samen met ketenpartners. Daarnaast lever je een actieve bijdrage aan vernieuwingen binnen de zorg en begeleid je cliënten en naasten waar nodig.
