Case Manager – Transitional Support Services (TSS)
High Point & Affiliated Organizations is seeking a Case Manager to join our team in New Bedford.
Position:
Part‑time, Monday–Friday (3‑7 PM or 4‑8 PM)
• Salary: $32.50–$48.50 per hour
- Maintain safety and security of residents and facility.
- Ensure timely contact of Program Manager/authorities in case of crisis or emergency.
- Follow referral procedures and schedule intakes for new residents.
- Facilitate intake process, set up new records, and complete intake paperwork.
- Develop initial Individual Service Plans (ISP) with residents.
- Make referrals for needed services to agencies such as DTA, SSA, DCF, medical/mental health providers, etc.
- Maintain accurate case files; obtain required authorizations and prepare case‑management notes.
- Update ISPs monthly and ensure timely evaluations.
- Record resident attendance at meetings, appointments, and groups.
- Meet with each resident bi‑weekly to review/evaluate progress, revise/update ISPs, and set new goals.
- Attend monthly case reviews with Program Manager.
- Participate in clinical supervision, staff meetings, and trainings.
- Ensure timely completion of admission/discharge satisfaction surveys.
- Provide house‑driving/transportation as needed.
- Perform all other duties as assigned by the Program Manager.
- High School Diploma or GED (or equivalent). One‑year experience in a substance‑use treatment field preferred.
- Associate’s degree or higher and supervised counseling experience in substance‑use treatment or closely related field preferred.
- Valid driver’s license.
- Courteous, professional demeanor with flexible, multi‑tasking ability.
- Excellent communication and organizational skills.
- Basic computer literacy and familiarity with software programs.
- Ability to collaborate and work cooperatively with others.
- Understanding of the relationship between substance use and mental health issues.
- Knowledge of therapeutic boundaries and ability to enforce rules respectfully.
- Compliance with federal regulations (42 CFR Part 2), confidentiality of substance use disorder, HIPAA, and TB/Flu/COVID‑19 assessment guidelines.
- Completion of HPAO‑approved de‑escalation program and CPR certification.
- Successful completion of all required training upon hire and annually thereafter.
