The Licensed Practical Nurse (LPN) provides each client with quality nursing and medical services within the scope of their license and under the supervision of the Nurse Manager. This person is a medical professional who actively supports and implements the established treatment goals and principles of the agency. The LPN treats children, adolescents, and adults within the context of recognizable and sound medical standards.
YCCS and ACCS are short‑term crisis stabilization units within Acute Care Services for children and adolescents up to age 18, and adults 18 and older. The primary function of CCS is to provide short‑term crisis stabilization, therapeutic intervention, and specialized programming in a staff‑secure environment with a high degree of supervision and structure.
Pay Range$30.00/hr – $33.00/hr
ScheduleSaturday 7am‑3pm (2 shifts), 3pm‑11pm;
Sunday 7am‑3pm;
Wednesday 3‑11pm (2 shifts). Must be available to work on‑call within a rotating schedule.
$1 per hour Language Differential (eligible languages include: Cape Verdean Creole, Haitian Creole, Portuguese, Spanish or ASL).
Job Responsibilities & Essential Functions- Fill physician orders
- Administer medication and engage in a medication reconciliation process
- Take/monitor vital signs
- Coordinate medical care
- Contribute to comprehensive assessment
- Brief crisis counseling
- Individualized crisis prevention planning
- Provide psychoeducation
- Assist with discharge planning and care coordination
- Monitor vital signs inclusive of withdrawal symptoms
- Work with Mental Health Counselors in ensuring an environment that promotes safety, recovery, and treatment
- Assist Nurse Manager in filling physician orders
- Participate in multidisciplinary team meeting
- Conduct Nursing assessments for incoming youth and adults
- Provide case consultation with clients, guardians, providers, and families of youth as indicated
- Must be available to work on‑call within a rotating schedule
- Maintain a high quality of work at all times
- Maintain a positive working relationship with agency staff, supervisors, and community contacts
- Maintain a level of professionalism
- Maintain regular and timely attendance
- Possess excellent communication, organizational and interpersonal skills
- Promote high staff morale and team atmosphere within department

Travel RequiredMust have a valid driver’s license and reliable transportation that meets CFS Driver Policy. Travel is required to provide services in different settings and to attend agency meetings, events, and trainings.
Additional InformationDue to the type of work and client needs, this position may need to work beyond a forty‑hour work week. Essential staff may be required to remain on shift or work an additional shift until relief staff are available. For overnight positions, staff are required to stay awake for the entirety of their shift. Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.
All 24‑hour/Acute Care Services program staff are considered essential, defined as personnel who are expected to report to work as scheduled, regardless of holidays and/or during any emergency situation and/or agency closing (i.e., inclement weather) to ensure the ongoing operations of the program.
