Hospice Registered Nurse Case Manager (RNCM)
Full Time Hospice Registered Nurse Case Manager (RNCM) for Castle Senior Living, Inc. Hospice coverage area includes Racine, South Milwaukee, and surrounding areas.
Position OverviewRNCM will provide skilled and compassionate nursing care and support to patients and families ponsibilities include:
- Conducting thorough skilled nursing assessments covering physical, psychological, social, spiritual, and environmental aspects.
- Identifying patient goals and appropriate interventions and creating individualized plans of care.
- Performing skilled nursing care in accordance with standards of practice, Castle Hospice policies, and the patient’s plan of care.
- Continuously reviewing and revising the plan of care based on patient and family needs.
- Coordinating schedules with the Hospice Director of Nursing or designee to ensure 24/7 patient coverage; participating in on‑call rotations that decrease as census grows.
- Current registration as a Registered Nurse in the state of Wisconsin.
- Minimum of one year of experience in medical/surgical, hospice, home health, or skilled nursing facility settings (preferred).
- Exceptional communication and problem‑solving skills with a compassionate bedside manner.
- Valid driver’s license and valid automobile insurance.
